Surgical treatment of obesity is used as a last resort, when other methods haven't worked. The main directions for bariatric surgery are Germany, Turkey, Switzerland, Austria and Israel.

What is bariatric surgery?

This is the branch of surgery that deals with the treatment of obesity (it does not include liposuction and abdominoplasty). The bariatric operations are to reduce the size of the stomach and the length of the small intestine in order to reduce the amount of food and absorption of nutrients.

When is bariatrics indicated?

According to international recommendations, bariatric operations are resorted to when the body mass index is 35 kg/m2, therapeutic methods of treatments do not help, and against the background of obesity, serious diseases such as diabetes mellitus, arterial hypertension, joint and vascular diseases, as well as obstructive sleep apnea have developed and progress. What types of operations are used in bariatric surgery?

Most often, gastric banding, sleeve gastroplasty, gastric bypass and biliopancreatic diversion are performed. Lately, these operations have been carried out by laparoscopic methods, which allow to do without large incisions and are less traumatic.

What is gastric banding?

This operation is believed to be the safest. A silicone band is placed around the upper part of the stomach. This allows to achieve weight loss by 50-60%. With sleeve gastroplasty, most of the stomach is removed. A thin tube (sleeve) with a diameter of 1cm is formed from the remaining part of the stomach. This creates an obstacle to the free passage of food. The small volume of such stomach contributes in fast saturation.

What can one do if one needs to reduce weight twice or more?

There are operations which reduce the volume of the stomach and shorten the small intestine. With gastric bypass, the stomach is sutured with titanium staples. It is divided into two sections: small one, with 50 ml volume, and large one (the rest of the stomach). Then the small intestine is connected to the small stomach in a short way. A significant portion of the food taken goes along the short way, thereby reducing the absorption of nutrients. Such operation gives 80% reduction in excess weight.

With biliopancreatic diversion the stomach is also sutured with the staples, and the small intestine is stitched to the resulting small stomach. The difference from the gastric bypass surgery is that only about 50 cm of the small intestine remains for the absorption of fats. The operation gives 90% reduction in excess weight.

Can one gain weight after bariatric surgery?

About 20% of patients do not lose 50% of their excess weight during a year after the operation. In addition, 30% of patients gain weight again during 2 years after the operation and 63.6 % — during 4 years. The operation is only part of the weight loss strategy. It is necessary to keep to the diet and follow the doctor's instructions, as well as to establish sleep and physical activity.

Can one drink alcohol after the operation?

You have to give up alcohol. After a bariatric surgery, absorption of alcohol changes as the number of enzymes, which break it down, decreases. Based on the studies, such patients have a higher alcohol level in blood than healthy patients with the same amount of alcohol consumed, and alcohol remains in their body longer.

Do operations affect the absorption of drugs?

With gastric bypass, this process is disrupted, and you may have to change the dose of the drugs taken.
